AnchorPoint Transitional Housing, Inc. is a Connecticut 501(c)(3) public charity providing safe, supportive transitional housing for adults experiencing housing instability, including individuals living with behavioral health needs. Through stable housing, individualized support, and strong community partnerships, we help residents build a foundation for stability, independence, and long-term well-being.

Our supportive housing model complements existing behavioral health and community-based services while helping residents develop the stability, skills, and confidence needed to move toward greater independence.

For many adults experiencing housing instability, securing a safe place to live is only one part of the journey. Long-term success requires stability, encouragement, meaningful community connections, and opportunities to regain independence.

AnchorPoint Transitional Housing was founded to bridge the gap between housing instability and long-term independence.

We provide more than a place to stay- we provide stability, structure, and support.

Residents are supported in establishing routines, strengthening life skills, connecting with community resources, and working toward greater independence.

Our approach is grounded in dignity, individualized support, and meaningful community connection.

AnchorPoint serves adults experiencing housing instability who would benefit from a structured, supportive living environment. Our residents may be transitioning from hospitals, behavioral health programs, homelessness, or other situations where safe housing is an essential step toward long-term success. Each referral is thoughtfully reviewed to ensure our program is an appropriate fit and that residents receive the support needed to thrive.

Jacqueline Blake, BSN, RN

Executive Director & Co-Founder

Jacqueline Blake is a Registered Nurse with experience in behavioral health, forensic mental health, clinical assessment, and care coordination. She leads the organization's development of a structured housing model designed to complement existing behavioral health services.
